Choose an evaluation type and creation method
Start with the evaluation type that matches the respondent, evidence and scoring workflow.
Evaluation type affects where a form can be assigned and how its results contribute to project comparison. Vendor questionnaires and RFIs collect vendor responses, demo scorecards collect observations, and stakeholder or use-case evaluations collect user testing evidence. After selecting the purpose, create from scratch, deploy a library template or use an assisted creation option when enabled.
Before you start
• Identify who will answer: vendor contact, project member, demo scorer or external stakeholder.
• Define whether answers should be scored and which project comparison stream should receive the result.
Step by step
1. Open evaluation creation
From the project, open Evaluations and choose New. Start here so the form is attached to the right project from creation.
2. Select the purpose
Choose the evaluation type that matches the workflow: vendor/RFI, demo scoring, stakeholder/use-case or another supported type. Do not choose by title alone.
3. Choose a creation method
Start from scratch for a unique form, deploy from the library for an approved reusable structure, or use the assisted method when available and appropriate.
4. Review copied or generated content
Treat templates and assisted drafts as starting points. Remove irrelevant questions, verify answer options and replace generic guidance with project-specific criteria.
5. Set identity and ownership
Use a clear title and description that distinguish the evaluation from other rounds or releases. Confirm the project and type before building deeply.
6. Continue to the builder
Add sections, questions, logic and scoring, then preview and publish. Creating the evaluation does not send it or make a draft visible to respondents.
What happens next
• Use the question builder and response sets that match the selected audience.
• Publish only after previewing the assignment path that will be used for that evaluation type.
Troubleshooting
The intended template is missing
Check that it is published or otherwise deployable, has the correct evaluation type and is available in this workspace.
An assisted creation option is unavailable
It may depend on workspace capabilities, plan or permissions. Create from scratch or deploy an existing template instead.
The form appears in the wrong workflow
Verify its evaluation type. Recreate or deploy it with the correct type rather than relying on a misleading title.
